Before your scheduled visit:

• Please print and complete both the patient information form and the medical history form that are attached, and bring them with you on your scheduled visit. To download the forms, click on the underlined links above.
• It is very important that your medical history is completed so your physician can spend quality time with you during your visit.
• Obtain any prior authorizations or referrals from your family or primary care physician before your appointment. It may be necessary to reschedule your visit without this authorization number if your insurance company requires it.

The day of your scheduled visit:

• Arrive 15 minutes before your scheduled appointment.
• Bring your completed patient information and medical history form
• Bring all of your medications that you are currently taking.
• We will need to make a copy of all insurance cards and your driver’s license. We will have to reschedule your appointment if you arrive without your current insurance or Medicaid card.
• Be prepared to pay for any unmet deductibles or co-pay amounts the day of your appointment. We accept cash, check, Mastercard, Visa, and Discover.
